Plan Guidelines
- Benefit coverage is subject to revision annually.
- Patients portion of bill is due at the time of service.
- You will not receive a membership card (your plan's effective date will be on file with the office).
- Patient must remain a plan member during the duration of treatment to retain plan benefits.
- NON-REFUNDABLE: No refunds or premiums will be issued at any time if the participant decides not to utilize the dental plan.
Note: These savings plans cannot be used in conjunction with another dental plan, dental insurance, PIP/Medical Claims, Workers Comp or financing programs such as Care Credit.
